Sometimes a tiny piece of the shell of another mollusk is inserted alongside the tissue sample. This is how pearls are really cultured by experts.  There is no grain of sand involved! 

Even natural pearls that have formed by accident don’t come from the mythical grain of sand but from a disturbance or break in the mantle layer of the shell that dislodges a few cells from the mantle tissue of the mollusk.  These cells then multiply and form a pearl sac.  The pearl grows inside this sac, as the mantle tissue continues to secrete pearl forming nacre in this enclosed area.  Any foreign object that has been introduced by the disturbance will be coated by the nacre, and the layers of nacre will form around it.  But sand is incapable of penetrating the oyster shell. Sometimes a parasite may do so.
