In the vasectomy reversal procedure, he will look for the two ends of the vas deferens after making two small incisions on both sides of the scrotum. Before performing the closure, Dr. Hickman will take a sample of the fluid from the testicular end of the vas and examine this under a laboratory microscope. Since the testicle is supposed to continue producing sperm even after the vasectomy, this sample fluid should contain some sperm. There are three possibilities when examining this fluid sample. The first possibility, which is actually the best possible situation, is that the fluid contains whole sperm. The second possibility, which is the most common, is that the fluid is thin and contains no whole sperm but only sperm parts. The third possibility is that the fluid is thick and pasty and contains no sperm.

The third situation is indicative of an obstruction that is known as an epididymal blowout. This means that the vas is obstructed at two points; one obstruction is at the vasectomy site and the other obstruction is a rupture of the tube in the epididymis. If the third situation is encountered, Dr. Hickman will need to perform a vasoepididymostomy in addition to a vasovasostomy. Fortunately for the patient, since the price quoted is an all-inclusive cost, there will be no additional cost if both repairs need to be performed during the vasectomy reversal.
